Serial NR. : PS2993H0RB
SIM Lock status : Locked (PH-NET PIN)

Check, if use latest dc-unlocker version.
Check, if autodetect was used.
Check, if modem software is not started or does not start at unlock time.
Disconnect all other modems and phones.
Check, if all needed drivers are installed.
Restart computer and do unlock again.

Sorry, do not know what may be wrong. Try to update firmware. Maybe this will help.
